After initial evaluation, therapy dog certification requires successful completion of a minimum of 8 weeks of observation and instruction in a therapeutic environment. The cost for TGOS Therapy Dog Program is $150. All fees are due at the first week of training and are non-refundable.

I agree to provide proof of current vaccinations and/or titers. I agree to follow the TGOS Therapy Program Guidelines. I understand that I am responsible for the behavior of my dog at all times and release The Gift of Sunshine, Inc. and its representatives from any and all liability for any injuries caused/sustained by me and/or my dog.
